Zufall Health's Delta Dental MobileThe Zufall Health Delta Dental Mobile is a licensed extension of Zufall Health. Schools in areas of high need throughout Morris, Sussex, Hunterdon, and Warren counties are able to request the van to visit their site to provide high-quality, preventive dental care.

To request a van visit, please email Michelle Blanchfield.

Eligibility: Children who are uninsured or have Medicaid or NJ Family Care are eligible to receive services on the van. Children with private insurance are not eligible. Pre-registration is usually required.

Appointments: Once a van visit is arranged with a school, the school nurse or a liaison at the site schedules the appointments a week before the event. The van/outreach staff provides information and a scheduling template to the school. In cases where the nurse or a liaison is not available, the Zufall Outreach staff will register the patients and schedule the appointments.

Services: The Zufall Health Delta Dental Mobile offers a range of preventive dental services for children up to 18 years old. Zufall Health receives DELTA DENTAL grant funding to cover co-pays for children’s van visits so all children’s services are at no cost to the parents!

First Visit: Typically during the first van visit to a school, children receive a dental screening exam, teeth polishing, and fluoride treatment at no cost to parents. In cases where a child has Medicaid, the services will be submitted for reimbursement.

Additional Services: A follow-up van visit can be arranged whereby identified children can receive sealants at no cost. A separate permission slip will be sent home with children who qualify after the first visit.

Registration: A brief registration form is available for the first van visit. Parents must provide medical history information and sign the consent form, but do not have to be present for treatment which saves them from taking time off from work!
