The Health, Education, and Legal Program (H.E.A.L.) is a medical-legal partnership between Legal Services of Northwest Jersey (LSNWJ) and Zufall Health. H.E.A.L. provides education, case management, and legal assistance to low-income Hunterdon, Morris, Somerset, Sussex, and Warren County residents.The program focuses on legal issues related to:

  • Public benefits including Social Security Disability Income (SSDI), Temporary Assistance for Needy Families (TANF), the Supplemental Nutrition Assistance Program (SNAP), unemployment benefits, NJ FamilyCare (Medicaid), Charity Care, and homelessness assistance
  • Domestic abuse, child custody, or child support/alimony
  • Tenants’ rights, evictions, home ownership issues, and Section 8
  • Financial protection
  • Advance planning and individual rights

The Maternal Wraparound Program (M-WRAP) provides intensive case management to residents of Morris, Sussex, and Warren counties who are pregnant or up to one year postpartum and are using or have recently used alcohol or drugs (including prescription narcotics). Services are provided during pregnancy and for up to one year after delivery. M-WRAP is funded by the New Jersey Division of Mental Health and Addiction Services.

M-WRAP staff is here to listen and help program eligible participants take the steps that are right for them and their baby to improve their health and well-being. Services are confidential and judgment-free.

At Zufall Health, promoting early literacy development is an important component of pediatric primary care. Through our participation in the Reach Out and Read New Jersey program, our providers give a new, age-appropriate book to pediatric patients at each well-child visit starting at six months of age. We also encourage parents to read to their child to create a language-rich home environment.

The Reach Out and Read program is based on research that frequent exposure to words through reading aloud helps young children develop language skills which serve as the foundation for all learning, and fosters a love of books that will last a lifetime.

Through our Arts in Health Program, we provided local professional and amateur artists the opportunity to showcase their creative talents and cultural diversity in a unique venue.

The Arts In Health program added another dimension to the care we provide by drawing upon the therapeutic value of art in the healing process. We believed fine art adds a sense of comfort and beauty to an atmosphere traditionally considered sterile, and helped alleviate the anxiety of being in a medical setting.

On a broader scale, our intent was to reach beyond our patients and staff to culturally enrich our community by providing new channels for creative expression and by inspiring an appreciation of the arts in individuals who typically may not have access to mainstream visual arts experiences.

Zufall’s Behavioral Health department offers two navigation programs for older adults in Morris County who need extra support with transportation, medical appointments, food access, and other essential services.

Older Adult Services provides case management to established Zufall Health patients age 65+ who live in Morris County, including:

  • Short-term or one-time assistance, including help scheduling appointments, setting up transportation, and accessing community resources
  • Assistance coordinating annual Medicare Wellness visits

Intensive Senior Support (ISS) is a program for older adults (60 years and over) who live in Morris County, providing:

  • A support system for vulnerable older adults who are at high risk of losing their independent living status due to mental illness, substance abuse, dementia, homelessness, domestic violence, and extreme resistance to services
  • Linkage for those in need of in-home services to supportive programs and access to comprehensive primary medical, dental, and behavioral health care to address chronic and acute medical issues.

ISS program participants will experience a continuity of care that facilitates stabilization of their medical and behavioral health conditions. Those who do not have a regular medical provider will be linked to Zufall Health for their primary care needs. Additionally, participants receive case management intervention services and appropriate referrals, including transportation to appointments to ensure continuation of care.

Financial support for ISS is provided, in part, by the Morris County Board of Commissioners.

At Zufall Health, we go beyond the walls of our health centers to meet people where they are. Our Community Outreach Program partners with local organizations, farms, shelters, housing authorities, and more to remove barriers to care and connect individuals and families to essential health services.

Outreach workers are at the heart of this effort. They help people navigate the health system, schedule appointments, coordinate on-site medical and dental care, arrange transportation, assist with translation and prescriptions, provide health education, and connect patients to community resources. Most importantly, they build trusting relationships—empowering our neighbors to take charge of their health.
