Integrated Mental Health Support
Behavioral health, or how we think, feel, and act, plays an important role in our overall health. That’s why Zufall Health integrates Behavioral Health services directly into primary medical care.
Our Behavioral Health services include mental health counseling and case management. Your primary care provider works closely with your behavioral health provider, case manager, or other member of the behavioral health team to monitor your progress and coordinate care.
Mental Health Counseling
Mental health counseling is offered in an outpatient setting as part of our primary care medical services. Counseling sessions are brief (20-30 minutes). Visits can take place in the office, or for added privacy and convenience, via telehealth.
Services are offered as part of primary care at Zufall Health. Existing Zufall patients seeking counseling can ask their medical provider for a referral or make a request by phone call, text message, or the Patient Portal. If you do not have a primary care provider, or you want to transfer to Zufall Health, Zufall has many locations to serve you. Learn more about becoming a new patient.
Zufall behavioral health providers follow a holistic, trauma-informed approach for counseling. Care considers your physical, emotional, and behavioral needs as well as your social environment and cultural identity. Counseling services can help you with:

Case Management
Case management services help patients navigate complex situations that may affect their health, such as housing instability, food access, transportation challenges, or connecting with outside behavioral health and social service agencies. Case managers work alongside behavioral health and medical providers to ensure patients have the resources and support needed to follow through with care.
Accessibility and Related Services
All services are offered in English and Spanish. A language line is also available as needed for patients who speak other languages.
Zufall offers limited psychiatric services. Please speak to your medical or behavioral health provider for a referral.
Patients in need of mental health and/or substance use services not offered through Zufall Health will be referred to other community agencies. In cases of emergency/crisis, patients are directed to call 988 or 911 or go directly to their local hospital’s emergency department.
