Anyone of any age, income, or insurance status can become a Zufall Health patient. Everyone is welcome.

Our complete list of accepted insurances includes Medicare, Medicaid (NJ FamilyCare), and many private insurance plans.

Patients with insurance will pay the co-pay listed on their insurance card.  

Patients without insurance will pay according to our sliding fee discount program, which ensures affordable access to all services. Fees are based on services received and are adjusted according to income and family size. Fees for medical services typically range from $25 to $80. Fees for dental services typically range from $45 to $200. Patients may have to pay separately for certain lab services, vaccines, and contraceptive supplies. No patient will be denied services due to inability to pay.

Patients with insurance may also participate in the sliding fee discount program if their co-pay is higher than the fee on the sliding fee discount schedule.

In-office and video or phone (telemedicine) appointments are available. There are several ways to make an appointment:

Yes, a limited number of walk-in appointments are available at each site. Please check the location’s hours before arriving. Since walk-in appointments are not guaranteed, we recommend making an appointment ahead of time, if possible.

In choosing Zufall as your medical home, you are encouraged to visit the same site and establish relationships with providers and staff. Seeing the same team helps us get to know you and provide more personalized care. However, you are free to visit any of our sites where your medical records are always available. Note that some locations offer different services than others. You can review the services offered by each location here.

Using the Patient Portal, you have easy and private access to your medical information online so you can view your personal health record whenever and wherever you have access to the Internet. You can also request appointments, medication refills, and referrals, view lab results, and communicate with providers and staff.

We offer telemedicine for all behavioral health services and many medical services such as allergies, asthma, colds and other viral infections, and medication changes. Telemedicine provides an easy and convenient way to receive care without coming to an office. Same-day telemedicine appointments are almost always available Monday through Friday. You can request a telemedicine appointment by calling one of our offices, texting 844-787-1846, or by using the Patient Portal, if you are a current patient.  

A Federally Qualified Health Center (FQHC), also known as a community health center, is a community-based organization that receives federal government funding to provide comprehensive primary care services to everyone, regardless of their income or insurance. FQHCs are overseen by the Health Resources and Services Administration, part of the U.S. Department of Health and Human Services, and must comply with the highest standards of clinical care. As private, nonprofit agencies, FQHCs are governed by a Board of Directors whose composition includes patients of the health center.

Zufall Health is a Federally Qualified Health Center covered under the Public Health Service (PHS) Act. This means that all health or health-related claims, including medical malpractice claims, are the responsibility of the United States Government. Zufall Health, its employees, and eligible contractors are considered federal employees and are immune from lawsuits. All filed claims are reviewed and/or brought before a court of law by the U.S. Department of Health and Human Services, Office of the General Counsel and the Department of Justice according to Federal Tort Claims Act (FTCA) requirements. 

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233 (g)-(n).

  • Plan to arrive 20 minutes before your in-office appointment to ensure you’ll be ready to see your provider at your scheduled time. 
  • Please notify us by calling the office or texting 844-787-1846 as soon as possible if you need to reschedule so we can offer that appointment slot to another patient. 
  • Be prepared to discuss if you have seen another doctor or been in the hospital or emergency room. 

At your first visit (and once every year after that), you must bring the following: 

  • Proof of identification. For infants or children, this can be a birth certificate or other ID. Parents accompanying children to pediatric appointments must also bring ID. 
  • Proof of address (If you rent a room or otherwise have no proof of address, you will be asked to complete Zufall’s Attestation of Address form). 
  • Proof of income such as a pay stub, your income tax return, or a letter of employment. If you have no income or if you are temporarily living with another person, please bring a letter indicating the support this person is offering you or use Zufall’s Statement of Support form. 
  • Immunization records (first visit only) 

If you are a new patient, you may wish to speed your waiting time by reviewing and completing the New Patient Registration Packet at home and bringing it with you to your visit. Before signing the consent form, please review the Notice of Privacy Practices (available in English and Spanish) and Patient’s Bill of Rights and Responsibilities. 

Yes. Zufall Health offers pharmacy services at Goodale Pharmacy. Goodale provides free delivery of medications to your home, workplace, or any Zufall medical location. You can switch to Goodale by talking to your provider or calling the pharmacy at 973-366-0976. To refill your prescription with Goodale, call the pharmacy use the RX365 app. 

First, you should check the label on your medication to see if you have any renewals left. If you do, you can call your pharmacy directly to order a refill. If you do not have any renewals left, you can call the health center to request that your provider authorize a renewal. If you are registered for the Patient Portal, you may request a renewal online by visiting the Patient Portal. Some medications may require that you see your provider to receive a renewal.

Zufall Health will call you to report your lab results as soon as they become available. Please be aware that some non-urgent test results may not be available for as long as two weeks. If you want to check on your test results, you may call the office. If you are registered for the Patient Portal, you may view your report there.

Patients can call any Zufall office after business hours. For non-urgent matters, leave a message and your call will be returned the next business day. If your need is urgent, you will be connected with our on-call medical provider. For extreme or life-threatening cases, call 9-1-1 or get help at the nearest hospital emergency room.
