In May 2026, Zufall Health’s first school-based health center officially opened to students, families, and the broader community in Franklin Township. Located on the campus of Franklin Middle School at 1 Berger Street, the facility is a collaborative partnership between Zufall Health, the Franklin Township Board of Education, and Franklin Township.

Who can use Zufall’s school-based health center?
The health center is available to anyone in the community, with a priority given to students and their families. The health center can serve as the primary point of care for a student or can work in conjunction with a student’s primary care provider (doctor, nurse practitioner, or physician assistant).

Why should families choose the school-based health center?
The health center offers convenient care that limits the time students are out of class and parents/guardians are away from work. In many cases, same-day or next-day appointments are available.

What services are offered?

• Well child visits (which meet the requirements of school, sports, camp, and employment physical exams)
• Routine lab tests, and immunizations
• Prescriptions and medication management
• Care for chronic conditions such as asthma and depression
• Care for acute injury and illness
• Reproductive health services, including pregnancy testing and birth control evaluation, dispensing, and management
• Testing and treatment for sexually transmitted infections
• Mental/Emotional health counseling, including evaluation, diagnosis, treatment, and referrals
• Comprehensive dental treatment, including but not limited to, dental screenings, routine cleanings, fluoride varnish application, sealants, and dental x-rays, and fillings
• Parent and child health education

To what extent are parents involved in care provided at the school-based health center? 
A parent or guardian must sign a consent form before their child may use most health center services. Once this is completed, the child may use the health center at any time. Supporting family communication is a principal goal of the health center. Health center staff encourage patients to discuss their health care with their parents/guardians. When clinically appropriate, Zufall staff will reach out to parents/guardians to share relevant visit information.

Are the services confidential?
Health care information is not shared without patient and/or parental/guardian permission. Information will be shared with the school only if you consent to sharing information. Information regarding a life-threatening situation will be shared as appropriate to ensure patient safety. New Jersey state law allows some visits to the health center to be confidential as appropriate. For example, adolescents 13 and up seeking reproductive health services and adolescents 16 and up seeking mental health counseling can request confidential visits without parental/guardian consent.

What are the costs?
We accept most forms of health insurance, including NJ FamilyCare (Medicaid), Medicare, and many commercial/private insurance plans. Patients who are uninsured will be screened for insurance eligibility. Any payments required for patients who are uninsured/underinsured will be on a sliding scale, adjusted based on income and family size. If parents/guardians are not present for the visit, we will not charge the student; we will notify the parent/guardian of the fees owed for the visit. Do you help with insurance enrollment?
When registering for use of the health center, families are asked to provide health insurance information or to apply for Medicaid, if eligible, or insurance through the NJ Marketplace. Insurance outreach and enrollment staff are available to assist families with the insurance application process.
